Page 1 is the Cancer Screening Leave Request. Please fill out the appropriate blocks online, print, sign it and submit to the Personnel Office. Page 2 is the Cancer Screening Verification Form which must be signed by the doctor. Also fill out the appropriate blocks online, then print and take to the appointment.
